How do I get a copy of the accident report?”

Answer:

Answer: If you have been directed to this site because you have scanned the QR Code on the Accident Information Exchange Form – please note that the form is not a crash report (TXDOT Crash Report CR3). The Information Exchange Form is a simple form that has been designed to expedite the exchange of necessary driver information in an effort to comply with the state law (TRC 550.023), while excluding as much of your personal information as possible.

You, or your insurance agent, may obtain a copy of the call log (Call Sheet) from records if you need police documentation of the accident. There will not normally be a TXDOT Crash Report (CR-3) associated with this incident if the Information Exchange Form has been given to you.
