CHECKING OUT A VIDEO:

1. The video must be checked out by a store manager or owner.
2. There is a $75.00 deposit required at the time of checkout (refundable if returned within 3 business days).
3. Videos are to be returned within 3 business days.
4. Instructions for viewing are given at the time of checkout.
5. You may call the Health Department to confirm that a video is available; however, videos cannot be reserved for a specific date.

RETURNING THE VIDEO:

1. Videos are due within 3 business days.
2. The following are required at the time the video is returned:
   A. Video
   B. Affidavit of attendees, signed by the manager who administered the test
3. The $75.00 deposit is returned at this time if the video is returned within 3 business days.
