Purpose of This Action Form
Any person may use this form to report problems experienced with accessing or using public facilities owned, constructed and maintained by the City of Mesquite. Public facilities include buildings, parks and open space where the public is invited, and the streets, sidewalks and other public accommodations within the City’s rights-of-way.
How to Submit This Action Form You may complete this form online and submit it to the Community Development Department by using the link below. The form may also be printed and delivered in person to the Building Inspection Division at the Municipal Services Building, 1515 N. Galloway Avenue.
What to Expect
You will receive a response to an Action Form within thirty days from the date you submit or hand deliver the form. If the City is responsible for correcting the problem you report, the response will include an estimated time frame for when you can expect the problem to be resolved.
